With BCBS, the prefix
decides, not the logo.
Blue Cross Blue Shield is not one payer; it is thirty-odd independent plans that honor each other’s cards. The behavioral benefit belongs to the member’s home plan, and the three letters before the member number are what tell you which one.
For a local Florida Blue member, eligibility is on Availity and the behavioral benefit is carved out to Lucet, verified through WebPass. Lucet also administers behavioral for Florida Blue Medicare Advantage, which is the part most practices miss.
For an out-of-state Blue arriving through BlueCard, the behavioral administrator is set by the member’s home plan and frequently is not the same as the local answer. Anthem members commonly route to Carelon. Drive off the alpha prefix, not the brand name on the card, because two cards with the same logo can carve out to two different vendors.
Local, out-of-state, and Medicare.
The Blues are the payer family where the routing question genuinely cannot be answered from the front of the card. Confirm eligibility first, read the prefix, and let the prefix tell you which behavioral vendor holds the benefit.
| Plan family | Step 1 · eligibility | Step 2 · behavioral | What to watch |
|---|---|---|---|
| Florida Blue, local members | |||
| Florida Blue commercial | Availity Essentials | Lucet (WebPass) | Formerly New Directions. Availity will show medical eligibility and a medical cost share; the behavioral answer is at Lucet. |
| Florida Blue Medicare Advantage | Availity Essentials | Lucet | Lucet administers behavioral for the Medicare Advantage line too, which is easy to miss when the plan is treated as ordinary Medicare. |
| Out-of-state Blues, BlueCard | |||
| Anthem and other out-of-state members | Availity, BlueCard | Depends on the member’s home plan | Read the alpha prefix and verify against the home plan. Anthem members commonly route to Carelon, but not always. |
| Any Blue where the prefix is unfamiliar | Availity, BlueCard | Confirm before assuming | When the prefix is not one you recognize, call the BlueCard line rather than guessing a vendor. A wrong guess produces a confident, wrong answer. |
| Adjacent | |||
| Curative | Availity Essentials | Not confirmed | Verify the behavioral administrator explicitly rather than inheriting the assumption from another Availity payer. |

The logo is the least informative thing on the card.
The numbers that actually reach a person.
Provider lines only. Lucet handles the behavioral benefit for Florida Blue; the BlueCard line is for members whose home plan sits in another state.
What to have in front of you.
