Where behavioral health
benefits actually live.

The insurance card tells you who covers the medical benefit. For behavioral health it is frequently wrong, and the claim is adjudicated by a company whose name is nowhere on it. This is our working map of who administers what, published.

How to use this

Every verification is two lookups.

Not one. The habit of treating eligibility and benefits as a single question is what produces most preventable behavioral health denials, because the two answers live in different systems and only one of them governs the claim.

Confirm the member is active
On the medical carrier’s portal, for the date of service rather than for today. This tells you coverage exists and gives you the plan type. It does not tell you what a therapy session costs.

Find who administers the behavioral benefit
Look the plan family up below. Where the benefit is carved out, verify again with that administrator: cost share, network status for the rendering clinician, prior authorization, visit limits and telehealth.

When the two disagree, the behavioral vendor wins
They adjudicate the claim. The medical portal is accurate about medical coverage and unreliable about behavioral health, and a confident wrong answer from the medical side is the most expensive result of the whole process.

Carve-out arrangements change at renewal, self-funded employer groups set their own, portals get renamed and phone trees get rebuilt. Confirm against the member’s card and the portal on the date of service.
