Blue Cross Blue Shield is not one payer; it is thirty-odd independent plans that honor each other’s cards. The behavioral benefit belongs to the member’s home plan, and the three letters before the member number are what tell you which one.

The short answer

For a local Florida Blue member, eligibility is on Availity and the behavioral benefit is carved out to Lucet, verified through WebPass. Lucet also administers behavioral for Florida Blue Medicare Advantage, which is the part most practices miss.

For an out-of-state Blue arriving through BlueCard, the behavioral administrator is set by the member’s home plan and frequently is not the same as the local answer. Anthem members commonly route to Carelon. Drive off the alpha prefix, not the brand name on the card, because two cards with the same logo can carve out to two different vendors.

The Blues are the payer family where the routing question genuinely cannot be answered from the front of the card. Confirm eligibility first, read the prefix, and let the prefix tell you which behavioral vendor holds the benefit.

Verifying the local plan for an out-of-state member
A BlueCard member walks in with a card that looks familiar, and the local portal returns an answer. That answer describes the local plan’s rules, not the home plan’s. The home plan adjudicates, and its behavioral vendor, cost share, authorization requirements and visit limits can all differ.
Treating Medicare Advantage as Original Medicare
Florida Blue Medicare Advantage carries a behavioral carve-out to Lucet. Original Medicare does not carve out at all. Treating the two the same way means checking the wrong entity and missing authorization rules that Original Medicare would not have imposed.
Calling Florida Blue about a behavioral question
The general Florida Blue provider line will route a behavioral question onward, and without the Availity transaction ID or your NPI in hand the call tends to dead-end. Take behavioral questions to Lucet directly, and have the transaction reference ready before you dial either one.

What to have in front of you.

The card, both sides
Front and back, as an image. On self-funded and third-party administered plans, the back of the card is often the only reliable routing information that exists.
Your NPI and tax ID, and the rendering clinician’s NPI
Behavioral network status is answered per clinician, not per practice. Asking about the group gets you an answer that may not apply to the person actually seeing the patient.
The CPT code and the date of service
Cost share, limits and authorization rules differ by service. Verify for the date of service, not for today, and for telehealth confirm both coverage and the expected place-of-service code.
A transaction or reference number, if the portal issued one
Some portals dead-end a call without it. And when a payer issues none at all, record that explicitly rather than leaving the field empty, so a later reader can tell the difference between “none exists” and “nobody checked.”
